Support4Me provides home supports to older people who are still managing well at home, but want some extra assistance, such as home help, delivered meals, personal care or home maintenance.
Many people use CHSP when they need low-level support. As their needs become more intensive or complex, requiring some case management assistance, Support4Me can assist with their Home Care Package application.
Support4Me is committed in ensuring that all older people living in our communities have equity and choice to achieve the potential in their lives and maintain independence.
This is achieved by providing supportive quality services, being knowledgeable, delivering on our promise and being responsive in a timely manner to meet the customer’s individual requirements.
Customers assessed as being eligible for services under the Commonwealth Home Support Program (CHSP) are eligible to receive services at a subsidised rate this assist in keeping fees reasonable and affordable.
You are expected to contribute to the cost of your service if you can afford to as per the schedule set below:
