What are CHSP Food services to you?

Are you finding it difficult to prepare meals?

Services may include providing meals at a community centre, helping with shopping for food, preparing and storing food in your home, and delivering meals to your home.

Support4Me will work with you to build on your independence by supporting your wellness and reablement goals with:

Assistance with preparing and cooking a meal in your home to improve your knowledge, skill, independence, confidence and safety; and
Advice on food including food preparation and nutrition, lessons, training and food storage and safety.

How is CHSP Food Services funded?

You will be asked to pay a small fee towards your services depending on you financial situation. This will be in accordance with Government requirements.

Support4Me believes in transparent fees that are all inclusive of case management, care coordination and care delivery.

Support4Me provides CHSP Food Services in the following NSW and North East Victorian local government areas:
